The Tokarski Home

When receiving hospice care in one’s own home is not an option, Willamette Vital Health offers the Tokarski Home for our hospice patients who need a place to call home in their final months of life.

Located at 1020 Elm Street NW, adjacent to Willamette Vital Health administrative offices, the home is licensed as a class 3 adult foster home by the state of Oregon. Individualized care is provided for up to 5 residents who are also enrolled as patients of Willamette Vital Health. Eligibility depends upon specific care needs and space availability.

Please call 503.588.3600 for complete eligibilty requirements and room & board costs. Private pay for room & board is on a sliding fee scale, depending on financial resources and ability to pay. Medicaid and insurance accepted, when applicable.
