Become a Community Hospice Liaison with Willamette Vital Health
The Role:
As a Community Hospice Liaison at Willamette Vital Health, you’ll be responsible for coordinating & generating hospice business in both existing and new accounts, identifying new markets and maintaining existing service as well as coordinating care for referred patients.
Your focus will be developing and sustaining strong working relationships in hospitals, facilities, and clinical settings with physicians, medical professionals, discharge planners, and others who refer or potentially refer to hospice and palliative care to increase referrals and admissions. Additional responsibilities include educating key constituents on the benefits of early and appropriate referrals to the company.
Requirements:
- Currently licensed as a nurse in the state of Oregon
- Associate or bachelor’s Degree preferred
- One (1) year of nursing experience in the local market with a strong knowledge of governmental regulations, Medicare requirements, and care plan development techniques.
- Must successfully perform and comply with all credentialing as required by hospital/healthcare facility.
- 2 years of experience in a clinical care setting (hospice preferred).
- 2 years of sales experience in a clinical care setting (hospice preferred).
- Experience in the hospice and palliative care industry is preferred.
- An acceptable combination of experience and/or training may be considered in lieu of formal education.
What You'll Do:
- Coordinates and facilitates referrals to hospice and palliative care based on admission criteria.
- Assist in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to home.
- Strong clinical foundation with the ability to understand and converse in clinical terminology required.
- Develop a consultative relationship in assigned accounts by making effective customer contacts and presentations.
- Develop and implement marketing and education programs to increase awareness of the company.
- Always maintains a high degree of confidentiality due to access to sensitive information.
- Follows all Medicare, Medicaid, and HIPAA regulations and requirements.
- Specialize in community specific culture and traditions to identify, develop, and maintain community support by providing education and outreach.
- Call on physicians, hospitals, nursing facilities and other healthcare providers (discharge planners, social workers, and health care coordinators) to promote WVH services.
- Act as company liaison to clients to resolve problems and provide information on services and maintain positive relations.
- Be an expert regarding the services provided by WVH ensuring effective communication and collaboration with providers.

Secondary Functions:
- Explain programs, obtain consents, and coordinate the admission process for referred patients.
- Participate in performance improvement projects (PIP).
- Review cost-effective options and prioritize approaches for optimal results.
- Participate in in-service education, orientation of team members, and agency programs.
- Engage in committees, groups, or focus groups.
- Foster a positive work environment.
- Contribute to the annual business development budget.
- Pursue professional growth and development through education programs.
- Adhere to professional ethics and integrate continuous quality improvement principles.
